Lost my tooth and had it put back in 10 years ago, and now it’s loose.

I am currently 24 years old. I was in a similar situation 10 years ago. My left front tooth was knocked out by someone, but it was still there. The root of the tooth apparently tore out the back of my gingival and the tooth was in one piece. My dentist put it back in and placed a cement mold on my uppers teeth. Then he did a root canal.

Now, fast forward: I was eating and heard a crack. (I assume it fractured the tooth?) And now my tooth has become loose. It’s like I can move it with my tongue. I am making an appointment to see my dentist. But, I have questions about the process. I want to know what the procedure will be and what my options are for replacing it.
